| 目的:探讨手术联合中药消肿汤治疗老年股骨粗隆间骨折对患者视觉模拟(VAS)评分、Harris 评分的影响。方法:选取2019 年2 月—2020 年8 月因股骨粗隆间骨折入院治疗的老年患者78 例，随机分为对照组、研究组；对照组患者行单纯股骨近端防旋髓内钉(PFNA)手术治疗，研究组患者则在PFNA 治疗基础上辅以中药消肿汤治疗。采用疼痛视觉模拟评分(VAS)对患者术后第1、3、5 和7 天疼痛情况进行评估，采用髋关节功能评分(Harris)对患者术前、术后1 周、4 周、8 周关节功能改善情况进行评估，同时对比两组术后消肿时间、骨折愈合时间。结果:术后1 d，两组间VAS 评分比较差异无统计学意义(P ＞ 0.05)，而术后3、5、7 d，研究组的VAS 评分均低于对照组，差异有统计学意义(P ＜ 0.05)；髋关节情况比较，术后1、4、8 周，研究组Harris 评分均高于对照组，差异有统计学意义(P ＜ 0.05)；研究组消肿时间、骨折愈合时间短于对照组，差异有统计学意义(P ＜ 0.05)。结论:在运用手术治疗老年股骨粗隆间骨折的同时辅以中药消肿汤治疗，有助于减轻患者术后疼痛、促进患肢关节功能的恢复，缩短康复进程，有较高的临床应用价值。 |

| Objective To investigate the effect of operation combined with Chinese medicine Xiaozhong Decoction on the Visual Analogue Scale (VAS) and Harris score of the elderly patients with intertrochanteric fracture. Methods A total of 78 elderly patients who were hospitalized for intertrochanteric fracture of femur from February 2019 to August 2020 were selected for this study. The patients were coded in Excel and then divided into control group and study group according to random number table method. Patients in the control group received simple proximal femoral anti-rotation nail (PFNA) surgery, while patients in the research group received PFNA treatment supplemented with traditional Chinese medicine Xiaozhong Decoction. Visual analog scale (VAS) was used to evaluate the pain of patients on the 1st, 3rd, 5th and 7th day after surgery, and hip function score (Harris) was used to evaluate the improvement of joint function of patients before surgery, 1, 4 and 8 weeks after surgery. Meanwhile, postoperative swelling reduction time and fracture healing time of the two groups were compared. Results There was no statistically significant difference in VAS scores among the groups on the first day after surgery (P > 0.05), while the VAS scores in the study group were lower than those in the control group on postoperative day 3, 5 and 7 (P < 0.05). The Harris scores in the study group were higher than those in the control group at 1st, 4th and 8th weeks after operation (P < 0.05). Meanwhile, time of swelling reduction and fracture healing in the study group were shorter than those in the control group (P < 0.05). Conclusion The treatment of intertrochanteric fracture of femur in the elderly with the treatment of traditional Chinese medicine Xiaozhong Decoction is helpful to reduce postoperative pain, promote the recovery of the affected limb joint function, shorten the rehabilitation process, which has high clinical application value. |
