基于 Caprini和 IMPROVE评分的分级护理预防腹部手术后深静脉血栓形成的临床观察

摘要:
目的:探讨 Caprini和 IMPROVE评分指导下的分级护理预防腹部手术后深静脉血栓形成的效果。方法:选取 2015年 8月—2018年 8月于滨州市沾化区人民医院以及济南市中医医院接受腹部手术的患者 98例,随机分为观察组( 44例)和对照组(54例),根据 Caprini血栓风险评估表和 IMPROVE出血风险评估表评估患者术后血栓风险等级和出血风险,观察组采取 Caprini和 IMPROVE评分指导下的分级护理,对照组采取常规护理,所有患者术后随访 6个月,统计患者术后深静脉血栓形成( DVT)发病情况。结果:观察组术后 DVT发生率低于对照组( 4.5% vs. 20.4%),差异有统计学意义(P< 0.05)。Caprini评分是患者术后 DVT发病的影响因素。结论: Caprini和 IMPROVE评分指导下的分级护理能够有效降低接受腹部手术治疗患者术后 DVT的发病率。

Clinical Observation on Prevention of Deep Venous Thrombosis after Abdominal Surgery by Graded Nursing Based on Caprini and IMPROVE Score

Abstract:
Objective To investigate the effect of graded nursing under the guidance of Caprini and IMPROVE scores in preventing deep venous thrombosis (DVT) after abdominal surgery. Methods Ninety-eight patients undergoing abdominal surgery from August 2015 to August 2018 in the People’s Hospital of Zhanhua District of Binzhou City and Jinan Municiple Hospital of Traditional Chinese Medicine were randomly divided into observation group and control group. According to the Caprini Thrombus Risk Assessment Form and the IMPROVE Bleeding Risk Assessment Form, the postoperative thrombus risk grade and bleeding risk were evaluated. The patients in the observation group were given graded nursing under the guidance of Caprini and IMPROVE scores, and the patients in the control group were given routine nursing care. All patients were followed up for 6 months, and the incidence of DVT was counted. Results Two of forty-four patients in the observation group were diagnosed of DVT while 11 of 54 in the control group were diagnosed of DVT. There was signi.cant difference in the incidence of DVT between the two groups after operation (P <0.05). Conclusion Graded nursing under the guidance of Caprini and IMPROVE scores can effectively reduce the incidence of postoperative DVT in patients undergoing abdominal surgery.
