软镜与微创经皮肾镜治疗 1~2 cm肾结石的疗效观察

摘要:
目的:比较输尿管软镜碎石术与微创经皮肾镜碎石术治疗 1~2 cm肾结石的疗效及术后疼痛程度,评估两者的安全性。方法:回顾性分析在我院接受手术治疗的 120例肾结石( 1~2 cm)患者,其中输尿管软镜碎石术(软镜组) 57例,微创经皮肾镜碎石术(MPCNL组)63例,比较两者的手术时间、术中出血量(按术后血红蛋白下降情况计算)、结石清除率、住院时间、术后疼痛程度、并发症及满意度的差异。结果:两组在年龄、性别、结石大小、手术时间、结石清除率、并发症上没有明显的差异。但 MPCNL组的术中出血量和住院时间高于软镜组,术后疼痛程度强于软镜组,患者满意度低于软镜组,差异有统计学意义( P<0.05)。结论:输尿管软镜碎石术治疗 1~2 cm肾结石在手术时间、结石清除率、并发症发生率上与微创经皮肾镜碎石术相当,但在术中出血量、平均住院时间、术后疼痛程度、满意度上明显优于微创经皮肾镜碎石术,可以考虑作为 1~2 cm肾结石治疗的首选。

Abstract:
Objective To compare the curative effect of flexible ureteroscopic lithotripsy and minimally invasive percutaneous nephrolithotomy in treatment of 1–2 cm kidney stones, and the differences in postoperative pain degree between the two groups, as well as to evaluate the safety of both. Methods Totally 120 patients with renal stones (1–2 cm) undergoing surgery in our hospital were retrospective analyzed and divided into .exible ureteroscopic lithotripsy group (group A, 57 cases) and minimally invasive percutaneous nephrolithotomy group (group B, 63 cases). Ultimately, the operative time, intraoperative blood loss (calculated by postoperative hemoglobin declined amount), stone-free rate, postoperative pain, complications, average hospitalization time and satisfaction were compared. Results There were no significant differences in age, gender, stone size, operation time, stone-free rate, and complications between groups A and B. However, there were differences in intraoperative blood loss, average hospitalization time, postoperative pain, and satisfaction, and the difference was signi.cant (P<0.05). Conclusions The .exible ureteroscopic lithotripsy has similar effect as minimally invasive percutaneous nephrolithotomy for the treatment of kidney stones about 1–2 cm in terms of average operative time, stone-free rate, and complications, but excels minimally invasive percutaneous nephrolithotomy in intraoperative blood loss, average hospitalization time, pain degree and comfort degree, which can be considered as the .rst choice for the treatment of 1–2 cm kidney stones.
