| 目的：探讨股前外侧穿支皮瓣修复术治疗手外伤组织缺损的效果、并发症及安全性。方法：将手外伤组织缺损患者进行分组治疗，对照组实施腹部带蒂皮瓣修复术，研究组实施股前外侧穿支皮瓣修复术，对比两组治疗效果与相关指标。结果：手术后研究组疼痛评分（2.11±1.21）与组织生长评分（1.63±0.19）优于对照组（3.98±1.32）、（0.89±0.32），研究组微炎症状态 CRP（3.36±1.12）mg/L、IL-6（167.34±31.34）ng/L、TNF-α(270.23±35.23) ng/L 优于对照组 CRP（6.98±1.63）mg/L、IL-6（201.54±34.54）ng/L、TNF-α(354.43±58.43) ng/L，创面愈合时间（5.43±1.92）d、抗感染药物使用时间（6.34±1.43）d、住院时间（19.43±2.52）d 等上均优于对照组（13.43±2.34）d、（11.43±1.78）d、（25.34±3.45）d ；研究组治疗总有效率 91%，高于对照组的 77% ；研究组感染率 14%，低于对照组的 26%（均 P<0.05）。结论：手外伤组织缺损实施股前外侧穿支皮瓣修复术，疗效显著，手部组织得到明显改善，疼痛有效好转，并发症少，治疗周期短。 |

| Repair of Hand Injury Tissue Defect with Anterolateral Thigh Perforator Flaps |

| Abstract: |
| Objective To investigate the effect, complications and safety of anterolateral thigh perforator ?ap in the repair of hand traumatic tissue defect. Methods The patients with hand trauma tissue defect were divided into control group and study group. The patients in control group were treated with abdominal pedicled flap and the patients in study group were treated with anterolateral thigh perforator flap. Results After operation, the pain score and tissue growth score of the study group were ( 2 . 11± 1 . 21 ) and ( 1 . 63± 0.19), which were better than those in the control group [(3. 98± 1. 32) and ( 0. 89± 0. 32)]. In the study group, the micro-inflammation status CRP was ( 3 . 36 ± 1 . 12 ) mg/L, IL- 6 was ( 167 . 34 ± 31 . 34 ) ng/L and TNF-a was (270.23±35.23) ng/L, which were better than those in the control group [(6.98±1.63) mg/L, (201.54±34.54) ng/L and (354.43±58.43) ng/L]. The time of facial healing was (5.43±1.92), the time of using anti-infective drugs was (6.34±1.43) d and the time of hospitalization was (19.43±2.52) d, which were all better than those in the control group [(13.43±2.34) d, (11.43±1.78) d and (25.34±3.45) d]. The total effective rate in the study group was 91%, higher than 77% in the control group, and the infection rate in the study group was 14%, lower than 26% in the control group (P<0.05). Conclusion The anterolateral thigh perforator ?ap is used to repair the tissue defect of hand trauma. The curative effect is remarkable, the tissue of hand is obviously improved, the pain is effectively improved, and the wound can be healed in a short time. |
