红外热成像在下肢动脉硬化闭塞症中的诊断价值

摘要:
目的:使用红外热成像技术将人体双下肢体表温度直观成像,通过观测并分析受试者大腿、小腿、足背、足趾及足底平均温度的数据图像,为红外热像仪对下肢动脉硬化闭塞症的诊断价值提供量化及可视化指标。方法:选取50例下肢动脉硬化闭塞症Ⅱ期和Ⅲ期患者和20例健康人对照组,运用红外热像仪采集双下肢红外图像进行对比分析。结果:正常人红外热像图双下肢皮温分布呈左右对称性;下肢动脉硬化闭塞症患者双下肢红外热像图分布不均匀,两侧热图不对称,远端不同程度低温改变,呈蓝色或蓝黑色;观察组患者大腿、小腿、足背、足趾、足底的平均温度均比健康人低,与对照组比较差异具有统计学意义(P<0.05);下肢动脉硬化闭塞症Ⅲ期患者患肢大腿、小腿、足背、足趾、足底分别较Ⅱ期患者的体表皮温低,其比较存在明显差异,差异有统计学意义(P<0.05)。结论:红外热像检查诊断下肢动脉硬化闭塞症,能够敏感显示患者双下肢的温度差异。且检查时患者无痛苦,疾病信息及图像易于储存。

Diagnostic Value of Infrared Thermography in Arteriosclerosis Obliterans of Lower Extremities

Abstract:
Objective Infrared thermography is used to image the body surface temperature of human lower limbs directly. The data images of the average temperature of subjects’ thighs, calves, back of feet, toes and soles were observed and analyzed. It provides quantification and visualization indexes for the diagnostic value of infrared thermography for lower limb arteriosclerosis obliteration. Methods Fifty patients with lower limb arteriosclerosis occlusion in Ⅱ period and Ⅲ period were selected and 20 patients were as healthy control group. The infrared images of the lower limbs were collected by infrared thermal imager for comparative analysis. Results The normal infrared thermography showed that the skin temperature distribution of the lower limbs was symmetrical. The infrared thermography of lower extremity in patients with lower extremity arteriosclerosis obliteration was not evenly distributed. Thermal images of patients with arteriosclerosis obliterans in the lower extremities showed varying degrees of blue or blue-black cold changes in the distal extremities of the lower extremities. The mean temperature of thigh, calf, back, toes and sole of the observation group was statistically significant compared with that of the control group (P<0.05). Patients with lower limb arteriosclerosis occlusion disorder in Ⅲ period had lower body skin temperature in limb thigh, leg, foot back, toes, and foot compared with patients in Ⅱ period, with statistically significant difference (P<0.05). Conclusion Infrared thermography can be used to diagnose arteriosclerosis obliterans of lower extremities, which can sensitively show the temperature difference of lower extremities. In addition, the patient suffered no pain during the infrared imaging examination, and the disease information and image of the patient were easy to be stored.
