| 目的 观察调补中州散联合肠外营养对老年胃癌术后患者肠道菌群、胃肠功能、营养状况的影响。方法:选取2021年2月—2023年2月于安庆市岳西中医院行胃癌手术的86例老年患者，根据随机数字表法分为治疗组(n=43)与对照组(n=43)。对照组于术后采取肠外营养支持，治疗组在肠外营养支持的同时，经鼻饲予以调补中州散治疗。记录并比较术后排气排便、肠鸣音恢复时间，比较两组患者术后第1天和第7天胃肠功能障碍(GFD)评分、肠道菌群计数和营养状况。结果:与术后第1天比较，术后第7天两组各检测指标均有改善(P＜0.05)。与对照组相比，治疗组术后第一次排气排便时间、肠鸣音恢复时间均缩短(P＜0.05)，术后第7天GFD评分降低，大肠杆菌和肠球菌计数减少，乳酸杆菌和双歧杆菌增加，血清生长激素(GH)、白蛋白(ALB)和转铁蛋白(TRF)水平升高，差异均有统计学意义(P＜0.05)。结论:调补中州散联合肠外营养能有效改善老年胃癌患者术后肠道菌群构成，有助于胃肠功能的恢复和营养状况的好转。 |

| Objective To analyze the effect of Tiaobu Zhongzhou decoction combined with parenteral nutrition on gastrointestinal function, intestinal flora, and nutritional status in elderly patients with gastric cancer after operation. Methods A total of 86 elderly patients undergoing gastric cancer surgery in Anqing Yuexi Traditional Chinese Medicine February of 2021 to February of 2023 were selected as research objects, and divided into treatment group (n=43) and control group (n=43) according to random number table method.All 86 patients were given routine measures according to their condition after operation.The control group received parenteral nutrition support on the 1st to 7th day after operation. The parenteral nutrition program of the treatment group was the same as that of the control group, and the treatment was supplemented with Tiaobu Zhongzhou decoction. The efficacy was evaluated after 1 week in both groups. The remission time of gastrointestinal dysfunction(GFD) related indicators, GFD score, intestinal flora calculation and nutritional status of elderly patients with gastric cancer after operation were compared between the two groups. Results Compared with the 1st day after surgery, the indexes of both groups were improved on the 7th day after surgery (P < 0.05). Compared with the control group, the first postoperative defecation time and bowel sound recovery time in the treatment group were shortened (P < 0.05), the GFD score on the 7th day after surgery was decreased, the count of Escherichia coli and enterococcus decreased, and the count of lactobacillus and bifidobacterium increased. Serum growth hormone (GH), albumin (ALB) and transferrin (TRF) levels were significantly increased (P < 0.05). Conclusion Tiaobu Zhongzhou decoction combined with parenteral nutrition on intestinal flora and nutritional status in elderly patients with gastric cancer after operation can improve the intestinal flora,and is helpful to improvement of the gastrointestinal function and nutritional status of patients. |
