消肿促愈汤溻渍对肛周脓肿术后疗效及对创面组织血管内皮生长因子、表皮细胞因子及pH值的影响

摘要:
目的:探讨消肿促愈汤溻渍对肛周脓肿术后疗效、创面组织血管内皮生长因子(VEGF)、表皮细胞因子(TGF-β1)及pH值的影响。方法:采用随机数字表法将2023年10月—2024年12月于本院就诊的123例肛周脓肿根治术的患者分为两组,观察组62例,对照组61例。对照组接受常规治疗+生理盐水溻渍治疗,观察组接受常规治疗+消肿促愈汤溻渍治疗,疗程为2周。比较两组患者的临床疗效、疼痛和水肿评分、创面修复情况(创面出血、创面渗液、创面修复率),以及创面组织生长因子(VEGF、TGF-β1)、pH值和不良反应发生率。结果:治疗14 d后,观察组的临床总有效率显著高于对照组(100.0% vs 80.0%)(P <0.05);治疗7 d、14 d后,观察组的创面疼痛、水肿评分显著低于对照组,创面出血、渗液评分均低于对照组;治疗14 d后,观察组创面修复率高于对照组(P <0.05);患者治疗7 d、14 d后,观察组TGF-β1、VEGF水平高于对照组,pH值低于对照组(P <0.05);治疗期间,两组不良反应发生率差异无统计学意义(P >0.05)。结论:消肿促愈汤溻渍疗法能够提高肛周脓肿患者术后临床治疗总有效率,促进创面修复,减轻患者疼痛,缩短治疗周期,且安全性好。

Curative effect of swelling-removing and healing-promoting decoction for medicated immersion on perianal abscess and its influences on vascular endothelial growth factor, epidermal cytokines and pH value in wound tissues

Abstract:
Objective To explore the curative effect of swelling-removing and healing-promoting decoction for medicated bath on perianal abscess and its effects on vascular endothelial growth factor (VEGF), transforming growth factor-β1 (TGF-β1) and pH value of wound tissues. Methods According to random number table, 123 patients undergoing radical surgery for perianal abscess in the hospital were divided into control group (n=61, routine treatment + normal saline immersion) and observation group (n=62, swelling-removing and healing-promoting decoction immersion added to control) from October 2023 to December 2024. All patients received for 2 weeks. The clinical efficacy, pain and edema scores, wound healing (bleeding, exudate, healing rate), wound tissue growth factors (VEGF, TGF-β1), pH values and adverse reaction incidence were compared. Results After 14 d of treatment, total clinical response rates was significantly higher in the observation group than the control group (100.0% vs 80.0%, P <0.05). After 7 d and 14 d of treatment, scores of wound pain and edema scores were significantly lower in the observation group than the control group, scores of wound bleeding and seepage were significantly lower in the observation group. After 14 d of treatment, the wound repair rate was higher in the observation group than the control group (P <0.05). After 7 d and 14 d of treatment, the levels of TGF-β1 and VEGF were higher in the observation group than the control group, and the pH value was lower in the observation group (P <0.05). During treatment, the difference in incidence of adverse reactions between the two groups was not statistically significant (P >0.05). Conclusion Swelling-removing and healing-promoting decoction for medicated immersion therapy can increase the total response rate of clinical treatment, promote wound repair, relieve pain, and shorten treatment cycle in patients with perianal abscess, with good safety.
