目的:研究中药贴敷疗法对阑尾切除术后愈合和切口感染的预防效果。方法:选取我院2019年2月—2020年2月间收治的120例急性化脓性阑尾切除术后患者,采用随机数字表法将其分为对照组和观察组,每组60例。对照组患者静脉滴注二代头孢加奥硝唑,切口以无菌纱布外敷。观察组患者在此基础上以芒硝加冰片入布袋行中药贴敷疗法。比较两组患者的切口愈合率、疼痛评分、拆线时间、红肿面积/切口长度、换药频率及切口不良愈合结局发生率,同时比较治疗前后的血清肿瘤坏死因子-α(TNF-α)、前列腺素E2(PGE2)及超氧化物歧化酶(SOD)水平。结果:观察组的手术切口甲级愈合率为96.67%,高于对照组的73.34%,差异有统计学意义(P=0.01)。观察组的术后VAS评分、拆线时间、红肿面积/切口长度、换药频率均明显低于对照组,差异有统计学意义(P<0.01)。观察组患者的切口感染和腹腔脓肿发生率明显低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的血清TNF-α、PGE2水平均明显低于对照组,血清SOD水平明显高于对照组,差异有统计学意义(P<0.01)。结论:芒硝加冰片贴敷疗法能明显提高阑尾切除术后切口愈合质量,降低切口感染发生风险,加快切口愈合和病情恢复,切口不良愈合结局发生率低。 |

Clinical Study on Treatment of Incision Infection after Appendectomy with Traditional Chinese Medicine |

Abstract: |
Objective To study the preventive effect of traditional Chinese medicine application therapy on healing and incision infection after appendectomy. Methods 120 patients with acute pyogenic appendectomy from February 2019 to February 2020 were selected, including 60 in each group. The control group intravenous the second generation of cefgaonidazole, and the incision was applied to sterile gauze. On this basis, the observation group were treated with traditional Chinese medicine application therapy. The incision healing rate, pain score, disassembly time, swelling area / incision length, drug change frequency, and adverse incision healing outcomes were compared. The serum tumor necrosis factor- α (TNF-α), prostaglandin E2 (PG E2) and superoxide dismutase (SOD) levels were compared before and after treatment. Results The surgical incision Grade A healing rate was 96.67%, which was significantly higher than 73.34% of the controls (P=0.01). Postoperative VAS score, disassembly time, swelling area / incision length and drug change frequency in the observed group were all significantly lower than those in the control group (P<0.01). The incidence of incision infection and abdominal abscess in the observed groups were significantly lower than those in the control group (P<0.05). After treatment, the serum TNF-α, PGE2 levels in the observed group were significantly lower than those in the controls (P<0.01), and the serum SOD levels were significantly higher than those in the controls (P<0.01). Conclusion Munro plus ice sheet patch therapy can significantly improve the quality of incision healing after appendectomy, reduce the risk of incision infection as well as accelerate the incision healing and the recovery of the condition. |
