尿液中基质金属蛋白酶-2及基质金属蛋白酶组织抑制剂-2在评价梗阻性膀胱顺应性改变中的临床研究

摘要:
目的:检测尿液中基质金属蛋白酶-2(MMP-2)及基质金属蛋白酶组织抑制剂-2(TIMP-2),探讨MMP-2及TIMP-2在诊断梗阻性膀胱顺应性改变中的应用价值。方法: 2016年11月—2020年1月期间前列腺增生致膀胱顺应性改变患者资料。研究组患者尿动力检查存在膀胱出口梗阻(BOO),且膀胱顺应性(BC)20 ≤BC<40 mL /cmH2 O;对照组为随机抽取前列腺增生引起BOO者,且BC ≥40 mL/cmH2 O。留取患者尿液并行血肌酐及胱抑素C检测,同时收集尿液于―80 ℃冻存待检测。尿样解冻后采用ELISA试剂盒测定MMP-2及TIMP-2尿液中含量。结果: 37例患者纳入研究组,两组间年龄、IPSS评分、体重指数(BMI)、膀胱过度活动症评分(OABSS)差异无统计学意义(P>0.05);除BC外两组间尿动力学参数(残余尿量、最大逼尿肌力、膀胱初感觉容量等)差异无统计学意义(P>0.05)。研究组尿液MMP-2及TIMP-2含量分别为(0.75±0.26)ng/mL、(359.8±123.7) pg/mL,高于对照组(0.52±0.21)ng/mL、(289.8±101.9)pg/mL,差异具有统计学意义(P<0.05)。结论:尿液中MMP-2与TIMP-2表达情况与梗阻因素导致的BC改变密切相关。

Clinical Study of Relationship between Urinary Matrix Metalloproteinase-2, Tissue inhibitor of Metalloproteinase-2 and Decreased Bladder Compliance by Obstruction

Abstract:
Objective To investigate the application value of matrix metalloproteinase-2 (MMP-2 ) and tissue inhibitor of metalloproteinase-2 (TIMP-2) in patients with bladder compliance changes caused by obstruction. Methods The clinical data of patients with bladder compliance changes caused by BPH from November 2016 to January 2020 were included in the study group, excluding neurogenic factors, urinary system tumor, diabetes, interstitial cystitis and other factors. Urodynamic examination of the enrolled patients indicated bladder outlet obstruction and 20 ≤ bladder compliance < 40 mL/cmH2 O. The control group was selected from patients with BPH caused by obstruction and BC ≥40 mL /cmH2 O. At least 20mL urine was collected and frozen at ―80 ℃for detection. Serum creatinine and cystatin were measured in each patient. The contents of MMP-2 and TIMP-2 in urine were determined by ELISA kit. Results 37 patients were included in the study group. There were no significant difference in the age, IPSS score, body mass index and overactivity bladder symptom score between the two groups. Except for bladder compliance, there were no significant difference in urodynamic parameters (residual urine volume, maximum urinary force, bladder initial sensory capacity) between the two groups (P > 0.05). The levels of MMP-2 and TIMP-2 in urine of the study group were (0.75±0.26) ng/mL, (359.8±123.7) pg/ mL, which were significantly higher than those of the control group (0.52±0.21) ng/mL and ( 289 . 8 ±101 . 9 ) pg/mL (P <0.05). Conclusion The expression of MMP-2 and TIMP2 in urine is closely related to the changes of bladder compliance caused by obstructive factors.
