液体伤口敷料联合负压滴灌治疗糖尿病足感染的临床效果观察

摘要:
目的:探讨普朗特液体伤口敷料联合负压滴灌治疗糖尿病足创面感染的效果。方法:选取2017年1月-2019年1月于北京中医药大学东直门医院周围血管科收治的糖尿病足患者80例,按照随机数字法分为观察组和对照组各40例,均接受标准化内科基础治疗,创面床准备完善后,观察组采用普朗特液体伤口敷料负压滴灌治疗,对照组采用康复新液负压滴灌治疗,记录两组治疗前、治疗后2周、治疗后4周创面症状积分、创面面积、血清IL-6、ESR及TNF-α水平。结果:治疗2、4周后,两组创面面积均较治疗前缩小,差异有统计学意义(P<0.01);治疗4周后,观察组创面面积及症状积分明显小于对照组(P<0.01)。治疗2、4周后,两组血清IL-6、ESR及TNF-α均较治疗前下降,差异有统计学意义(P<0.01)。治疗2、4周后,观察组的IL-6、TNF-α及ESR水平均低于对照组,差异有统计学意义(P<0.05)。结论:普朗特液体伤口敷料负压滴灌治疗能够减轻糖尿病足创面感染,改善临床症状,促进伤口愈合。

Clinical Observation of Liquid Wound Dressing Combined with Negative Pressure Drip Irrigation in the Treatment of Diabetic Foot Infection

Abstract:
Objective To investigate the effectiveness of liquid wound dressing combined with negative pressure drip irrigation in the treatment of diabetic foot wound infections. Methods 80 cases of diabetic foot patients admitted to the peripheral vascular department of Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine from January 2017 to January 2019 were selected and divided into 40 cases each in the observation group and control group according to the random number method, all of which received standardised basic medical treatment. The control group was treated with negative pressure drip irrigation with a new solution of rehabilitation, and the points of wound symptoms, wound area, serum levels of IL-6, ESR and TNF-α were recorded before treatment, 2 weeks after treatment and 4 weeks after treatment in both groups. Results After 2 and 4 weeks of treatment, the wound areas of both groups were smaller than those before treatment, and the difference were statistically significant (P<0.01). After 4 weeks of treatment, the trauma area and symptom points of the observation group were significantly better than those of the control group (P<0.01). After 2 and 4 weeks of treatment, serum IL-6, ESR and TNF-α decreased in the two groups compared with those before treatment, and the difference was statistically significant (P<0.01). After 2 and 4 weeks of treatment, the levels of IL-6, TNF-α and ESR in the observation group were all smaller than those in the control group, and the difference was statistically significant (P<0.05). Conclusion Liquid wound dressing with negative pressure drip irrigation can reduce diabetic foot wound infection, improve clinical symptoms and promote wound healing.
