核转录因子 κ B、P53在膀胱癌组织中的表达及其与临床病理特征的关系

摘要:
目的:探讨核转录因子 κ B(NF-κB)、P53蛋白在膀胱尿路上皮癌组织中的表达及其与患者临床特征及术后复发的关系。方法:选择 2014年 2月—2017年 1月成都市郫都区人民医院确诊并手术的膀胱移行上皮癌患者 48例、同期手术治疗的非膀胱癌患者 16例,采用免疫组织化学 SP法检测膀胱癌组织、癌旁组织及非膀胱癌患者正常尿路黏膜组织中 NF-κB和 P53蛋白的表达水平。探讨 NF-κB和 P53蛋白阳性表达与患者临床特征包括年龄、性别、病理分级、临床分期、淋巴结转移及术后复发情况的关系。结果:膀胱尿路上皮癌组织、癌旁组织和正常尿路黏膜组织中 NF-κB蛋白阳性表达率分别为 52.5%(30/48)、16.7%(8/48)和 6.3%(1/16);P53蛋白阳性表达率分别为 72.9%(35/48)、45.8%(22/48)和 12.5%(2/16),癌组织>癌旁组织>正常尿路黏膜组织( P<0.05);NF-κB阳性表达与膀胱尿路上皮癌患者肿瘤病理分级存在相关性,高级别膀胱尿路上皮癌( G2/G3)组织中 NF-κB阳性率显著高于低级别( G1)肿瘤组织( P<0.05);P53阳性表达与膀胱尿路上皮癌患者肿瘤分期存在相关性, T2-3期患者阳性率显著高于 Tis-1期(P<0.05);NF-κB表达阳性和阴性患者的中位无疾病进展生存时间( DFS)分别为 8.9个月和 30.1个月,阳性组复发风险显著高于阴性组( P<0.05);P53表达阳性和阴性患者的 DFS分别为 9.2个月和 28.6个月,阳性组复发风险显著高于阴性组( P<0.05)。结论:与正常膀胱黏膜比较,膀胱尿路上皮癌患者 NF-κB、P53蛋白表达水平显著上调,并与患者肿瘤分级和分期有关,可作为膀胱尿路上皮癌术后复发预测的分子标志物。

Abstract:
Objective To investigate the expression of NF-κB and P53 in bladder urothelial carcinoma and the relationship with patients' clinicopathological features and prognosis. Methods Forty-eight patients with bladder urothelial carcinoma treated with surgery and 16 non-bladder cancer cases were included in this study from February 2014 to January 2017 in Pidu District People' s Hospital of Chengdu. The expression levels of NF-kB and P53 protein in bladder urothelial carcinoma, adjacent normal bladder tissues of bladder urothelial carcinoma patients and normal urinary tract mucosa of non-bladder cancer patients were examined by immunohistochemical SP assay. According to NF-kB and P53 protein expression, the bladder urothelial carcinoma patients were divided into positive and negative expression group. The relationship between NF-kB, P53 protein expression and patients’ clinical characteristics (including age, sex, pathological grade, clinical stage, lymph node metastasis and recurrence after operation) was evaluated. Results The positive expression rates of NF-kB protein were 52.5% (30/48), 16.7% (8/48) and 6.3% (1/16) for bladder urothelial carcinoma tissue, corresponding tissue and normal urinary tract mucosa respectively. For P53, the positive rates were 72.9% (35/48), 45.8% (22/48) and 12.5% (2/16) for the above three types of tissues. The positive expression rates of NF-kB and P53 had statistical difference among the three types of tissues (P<0.05). The positive expression of NF-kB in high grade bladder urothelial carcinoma (G2/G3) was significantly higher than that of low grade one (G1) (P<0.05); The positive expression of P53 was correlated with the stage of bladder urothelial carcinoma, and the positive rate of T2–T3 was signi.cantly higher than that of Tis-T1 (P<0.05); Disease free survival (DFS) was 8.9 months and 30.1 months respectively for NF-κB positive and negative group, which indicated the recurrence risk of positive group was signi.cantly higher than that of negative group (P<0.05); DFS of P53 positive and negative patients was 9.2 months and 28.6 months, respectively. The recurrence risk of positive group was also significantly higher than that of negative group (P<0.05). Conclusion Compared with normal bladder mucosa, the expression of NF-kb and P53 protein in patients with bladder urothelial carcinoma is signi.cantly increased, which is related to the grade and stage of bladder cancer, and can be used as a molecular marker for predicting the recurrence of bladder urothelial carcinoma after operation.
