| 目的：探讨多向锁定髓内钉固定术治疗股骨远端骨折的疗效。方法：选取股骨远端骨折患者 80 例，依据治疗方法分为多钉组（n=30 例）和逆钉组（n=50 例），逆钉组给予逆行髓内钉固定术治疗，多钉组给予多向锁定髓内钉固定术治疗，比较两组术中出血量和手术时间、完全负重时间、骨折愈合时间及关节功能、并发症等。结果：多钉组术中出血量 [(276.83±31.43) mL vs (353.18±40.37) mL]、完全负重时间 [(9.76±1.34) 周 vs (11.87±2.15) 周 ]、骨折愈合时间 [(12.43±1.47) 周 vs (15.43±1.75) 周 ]、并发症发生率 (6.67 % vs 16.00 %) 明显低于逆钉组（P<0.05），2 组手术时间[(68.24±10.57) min vs (66.42±10.28) min] 基本相同（P>0.05）；多钉组功能恢复优良率 (96.67 % vs 84.00 %) 明显高于逆钉组（P<0.05）。结论：多向锁定髓内钉固定术可有效减少股骨远端骨折术中创伤和并发症，有利于缩短患者完全负重、骨折愈合时间，并可有效促进关节功能的恢复。 |

| Objective To discuss the curative effect of multidirectional locking intramedullary nail ?xation in the treatment of distal femoral fracture. Methods Eighty patients with distal femur fracture were selected, and according to the treatment method, they were divided into multi nail group (n = 30) and inverse nail group (n= 50). The patients in inverse nail group were given retrograde intramedullary nail fixation, and the patients in multi nail group were given multidirectional locking intramedullary nail fixation. The intraoperative blood loss, operation time, full weight bearing, fracture healing time, joint function, and complications were compared between the two groups. Results The intraoperative blood loss [(276.83±31.43) mL vs (353.18±40.37)mL], full weight-bearing time [(9.76±1.34) weeks vs (11.87±2.15) weeks], fracture healing time [(12.43±1.47) weeks vs (15.43±1.75) weeks], complications rate (6.67 % vs 16.00%) of multi nail group were signi?cantly lower than those of inverse nail group (P <0.05), the operation time [(68.24±10.57) min vs (66.42±10.28) min] of multi nail group and inverse nail group were basically the same (P>0.05). The function recovery excellent rate (96.67% vs 84.00%) of multi nail group were signi?cantly higher than those of inverse nail group (P<0.05). Conclusion Multidirectional locking intramedullary nail fixation can effectively reduce the intraoperative trauma and complications in patients with distal femoral fractures, it is helpful to shorten the full bearing and fracture healing time of the patients, and it can effectively promote the recovery of joint function, which is worth for further clinical promotion. |
