不同剂量羟考酮对全麻术后早期瑞芬太尼痛觉过敏的影响

摘要:
目的:观察羟考酮对全麻术后早期瑞芬太尼痛觉过敏的影响及最佳剂量。方法:选择择期全麻下开腹行子宫全切术患者 50 例,随机分为 A、B、C、D、E 5 组,每组 10 例。A 组为空白对照组,B、C、D、E 组在手术结束缝皮时分别静脉给予羟考酮 0.05、0.1、0.15、0.2 mg/kg。记录 5 组患者苏醒时间(拔除气管导管时间),恶心、呕吐、躁动、呼吸抑制、皮肤瘙痒等不良反应发生例数及给药后 1、2、4、12、24 h 舒芬太尼的用量。结果:E 组术后苏醒时间(21.8±7.2)min, 明显长于 A、B、C、D 组(P<0.05);D、E 组术后不同时间点舒芬太尼的用量分别为(51.8±3.3)μg 和(52.6±3.5) μg,低于 A、B、C 组(P<0.05);E 组不良反应发生例数 5 例,多于 A、B、C、D 组(P<0.05)。结论:全麻患者术毕前静注合理剂量羟考酮,可以有效预防术后早期瑞芬太尼诱发的痛觉过敏反应,不延长患者的苏醒时间,不增加不良反应发 生率,推荐剂量 0.15 mg/kg。

Abstract:
Objective To observe the effect of different doses of oxycodone on postoperative hyperalgesia induced by remifentanil in general anesthesia patients, and select the best dose. Methods Fifty cases of elective hysterectomy under general anesthesia were randomly divided into five groups of A, B C, D and E, n=10. A group is control group, and the patients in B, C, D and E groups were injected with oxycodone (0.05, 0.1, 0.15 and 0.2 mg/kg).The recovery time and number of adverse events such as nausea, vomiting, dysphoria, respiratory inhibition, pruritus, and sufentanil consumption at the time of 1, 2, 4, 12 and 24 h after operation were recorded. Results The recovery time of E group was (21.8±7.2) min, longer than that of A, B, C and D groups (P <0.05). The sufentanil consumptions of D and E groups were (51.8±3.3) μg and (52.6±3.5) μg, lower than those of A, B and C groups (P<0.05). The number of adverse reactions in E group was 5, more than that of A, B, C and D groups (P<0.05). Conclusion Rational dose of oxycodone can effectively prevent remifentanil-induced hyperalgesia in early postoperative patients after general anesthesia, without prolonging the recovery time or increasing the incidence of adverse reactions, and the recommended dose is 0.15mg/kg.
